Community Nursing Services
We provide individuals with nursing care and support for personal care and daily living activities when they are no longer able to perform these activities on their own. These services help people to stay in their own homes rather than go to a hospital or long term care facility when they need nursing care or help with daily living activities because of age, disability, injury, or illness. Our experts provide care to clients who may need acute, chronic, palliative or rehabilitative care. The main goal is to teach you and your caregivers how to manage your own health care so that you can live as independently as possible.

- Assist people in remaining as independent as possible.
- Support families in coping with a family member's need for care.
- Help people stay at or return home and receive needed treatment, rehabilitation or palliative care.
